Participant Waiver and Consent Form

Athletic Arts Academy LLC requires participants and their guardians (if under 18) to acknowledge and agree to the following terms and conditions regarding participation in Academy activities, including classes, events, and performances.

1. Assumption of Risk and Health Liability

By participating in classes, events, and performances at Athletic Arts Academy, I acknowledge and understand that: - Participation in physical activities such as dance, cheer, and other sports may involve inherent risks, including but not limited to falls, sprains, strains, and other injuries. - I (or my child) am voluntarily engaging in these activities and assume all risks associated with participation. - I certify that I (or my child) am in good physical health and have no conditions that would prevent safe participation in these activities. - I agree that any injuries sustained during participation are my (or my child’s) responsibility and will be covered by my personal insurance. Athletic Arts Academy does not provide accident or health insurance for participants.

2. Release of Liability

I agree to release, waive, and hold harmless Athletic Arts Academy LLC, its instructors, staff, and representatives from any and all claims, demands, or causes of action arising out of or related to participation in Academy activities, including any claims of negligence resulting in injury, illness, or other damages.

3. Consent to Photograph and Record

I authorize Athletic Arts Academy to: - Photograph, video record, and capture audio of me (or my child) during classes, events, and performances. - Use these materials for promotional, educational, and informational purposes, including but not limited to: - The Academy's website - Social media platforms (e.g., Facebook, Instagram, YouTube) - Printed promotional materials (e.g., flyers, brochures, newsletters) - Posters and other official Academy communications I understand that these materials are the property of Athletic Arts Academy and waive any rights to inspect, approve, or claim compensation for their use.

4. Duration of Agreement This waiver and consent will remain in effect indefinitely unless written notice of revocation is provided to Athletic Arts Academy. Any revocation will not affect materials or claims arising before the date of revocation.

Acknowledgment and Agreement I have read and understood the terms outlined in this waiver and consent form. By signing, I confirm my agreement to the terms and conditions set forth by Athletic Arts Academy LLC.
